Site Onboarding Tasks
No less than six weeks prior to the start of the rotation, please contact the site coordinator with the following information:
- Full name including middle name
- Date of Birth
- SSN
PLEASE NOTE: You may call the site coordinator to provide this information if you are not comfortable sharing your personal identifiable information via email. This information is necessary to set up your hospital privileges for your rotation.
TB Testing
This site may require proof of a negative TB test(s). Even though this is no longer a requirement of the UWSOM, students are required to comply with any site specific TB testing requirements. Please contact the site coordinator to inquire if a TB test is required.
