
Cognition in Primary Care & Project ECHO Dementia are offering grants of $4,000 each (up to $8,000 combined) to Federally Qualified Health Centers, Rural Health Centers, or other eligible clinic systems for their participation in this program. This grant is intended to compensate Health Centers for the time they spend in service of this program. Health Centers may apply to one or both programs, depending on the needs of their system.

Purpose
Under guidance from the Centers of Disease Control's BOLD Grant Program, Cognition in Primary Care and Project ECHO Dementia have partnered with the shared goals of improving primary care in Washington State for people living with Dementia and Mild Cognitive Impairment by offering grants to Federally Qualified Health Centers (FQHCs), Rural Health Centers (RHCs), and other similar clinic systems for their involvement in one or both programs.
Cognition in Primary Care and Project ECHO Dementia offer different methodologies to achieve these shared goals; and focus on different levels of involvement.
Cognition in Primary Care (CPC) offers training, education and practical tools providers can use in clinic to improve their practice. Developed by primary care, for primary care, CPC has been streamlined, tested, and shown to be an efficient and effective as an easy-lift mechanism for quality improvement.
Project ECHO Dementia is focused on practice transformation through ongoing training and case presentations; wherein providers can learn in community with each other and bring forth examples from their clinical experience. The ultimate goal of Project ECHO Dementia is developing practitioners within clinic programs who specialize in Dementia care and can advise and assist others. Leadership in both projects have worked closely to consider how these programs can work together effectively and synergistically; and with assistance from the BOLD Grant; are excited to extend these programs into health systems that can benefit.

This grant is intended to compensate Health Centers for the time they spend in service of this program. Health Centers may apply to one or both programs, depending on the interest and needs of their system. In exchange, health systems are expected to make the appropriate staff available as needed to fulfill the requirements of each program (please see the documents for additional information) and offer data to UW Medicine to measure the impact of each program.
Applicant Eligibility
This application is open to organizations, applicants, or teams licensed or capable of becoming licensed to do business in the state of Washington, who are available for work, and who are Federally Qualified Health Centers, as defined by the Health Resources and Services Administration (HRSA) and Rural Health Centers, as defined by the Centers for Medicaid and Medicare Services (CMS), and other similar clinic systems.
